Social Worker I
The BVI Health Services Authority (a client-centered healthcare organization) invites qualified and professional applicants for the position of Social Worker I. The Social Worker I provides persons, families, or vulnerable populations with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care-givers, providing patient education and counseling, and making necessary referrals for other social services. Ensures that comprehensive documentation is completed for all contacts.
PRIMARY DUTIES & RESPONSIBILITIES (included but not limited to):
Case Management:
(1) Conduct social investigations of patients upon request or when deemed appropriate and employ the necessary interventions
(2) Act as advocate for elderly patients and other referred clients/patients.
i. Develop and organize policies and programs for elderly (65+) population within the BVI Health Services Authority
ii. Assess for elder abuse or neglect and advocate for patients’ best interest.
iii. Coordinate home visits and home care arrangements prior to patient discharge from Peebles Hospital
(3) Participate in physician ward rounds weekly - Tuesdays: Surgical team; Medical team; OB and Psychiatry as needed
(4) Liaise with the multi-disciplinary treatment team members in the best interest of clients
(5) Liaise with the Social Development Department and other support agencies as needed to aid in the execution of holistic patient care
(6) Co-ordinate patients’ referrals from Peebles Hospital to local and overseas providers or specialists
(7) Coordinate medical evacuation for patients appropriately referred for this service (transfer to other medical facilities) communicates with NHI on behalf of in/out patients.
(8) Organize and facilitate family meetings for the sharing of information and decision making as it relates to patients’ care
(9) Initiate investigation of referred or suspected child abuse or neglect cases and take authorized protective action when necessary.
(10) Conduct group and individual counseling when necessary as it pertains to the execution of holistic patient care
Community
(11) Maintain regular follow-up of vulnerable patients discharged into the community through weekly phone assessments and quarterly/ or as the need arises for home
assessments
(12) Communicate with patients, caretakers, family members and district nurses in order to facilitate regular medical checks and appointments as it relates to discharged
vulnerable patients’ plans of care in the community
(13) Liaison with Community Health and Mental Health Services for the assessment, intervention and monitoring of patients seen in the community as needed; and maintain
weekly and monthly follow up
Support
(14) Is an active team member of the BVI Crisis Intervention Team
Education
(15) Participate in educational programmes offered or arranged by the institution, relevant to the needs of the client system and carry out relevant research activity.
(16) Participate in regular continuing education activity relevant to evidenced based social work practice
Administrative
(17) Collect, record, store and maintain patients’ records in a timely fashion, with due regard for confidentiality
(18) Assist with maintaining the inventory of office supplies by tracking and ordering supplies in a timely manner
Other
(19) Advocate for patient voice and choice
(20) Any other duties consistent with role profile as assigned by the Supervisor
Minimum Requirements:
Education & Experience
v Bachelor of Arts/Science Degree in Social Work OR
v Bachelor of Science Degree in a related field, with one (1) year certificate in Social Work.
v Case management experience including interviewing, assessments, counseling and social intervention techniques
v At least two years working experience as a trained social worker; experience working in a health institution, especially a hospital, would be an asset.
Licenses and/or registrations
v Must meet local certification requirements.
v Qualifies MSW, LGSW

v Knowledge of the environment, and community resources and the ability to mobilize same.
v Ability to plan and organize work.
v Ability to relate to individuals and groups of varying socio-economic and educational background.
