1. JOB SUMMARY: To provide pharmacy services to patients within the Cheshire Hall Medical Centre and Cockburn Town Medical Centre on a rotational basis between the Pharmacy posts.

The post holder will do or cause to be done the activities below.  These activities are not exhaustive and other duties which are within the realms of his/ her qualifications, experience and ability may be required.

  1. Clinical

 

1.1  Interprets medication orders/prescriptions and dispenses or supervise the dispensing of    medications in accordance with legal, professional, and departmental policies, procedures, and standards.

1.2 Reviews prescriptions for therapeutic suitability by referring to standardised references, interpreting laboratory data, and/or reviewing patient specific demographics. Communicates with physicians, patients, and/or other healthcare team members for prescription clarification and problem resolution. Documents the outcome of any interaction with the prescriber in the patient’s health record.

 

1.3  Minimizes risk to patient, public and other healthcare professionals by giving appropriate advice on drug handling, administration and disposal.

1.4  Provides medication counselling regarding drug therapy to patients and/or their families, as required.

1.5  Participates in ward rounds, team meetings and provision of pharmacy services to enhance patient care, as required.

1.6  Design and implement standards of practice regarding  the medication use process. This involves standards for prescribing,dispensing administiring,and monitoring

 

  1. 2.    Administrative

 

2.1.  Creation,Review & Maintenance of Policy & Procedures

Participate with colleagues and stakeholders  in the development, review and maintenance of policies and procedures for the pharmacy department.

              

 

2.2.  Adherence to Policy & Procedures

 

              Adhere to policies and proceedures

      2.3  Assist the organization with the selection and procurement of Pharmacy  equipment.

2.4 conduct and or participate in performance management

 

2.5  Maintain adequate inventory and facilitate the procurement process in collaboration with the purchasing department;

2.6  Ensures proper storage and safe custody and control of all drug supplies within the institution

2.7 Ensures when necessary the safe, effective and efficient transportation of pharmaceuticals stock between the two hospital sites.

2.7  ‘Manages cost’ and utilization of pharmaceuticals to ensure their safe, effective and economic use.

2.8  Maintains all required records for narcotic and controlled drugs, special access programme drugs and departmental statistics in accordance with legal, professional and departmental standards, polices and procedures.

2.9  Prepare and submit reports as required.

2.10       Participates in the development, coordination and implementation of staff orientation, acts as role model and mentor.

2.11       Participates in quality improvement and risk management activities in accordance with established policies and procedures.

2.12       Performs other professional duties as may be assigned within his/her scope and ability

3.1.  Membership of  Committees

 

The post holder may be required to sit on committees either as a routine or as ad hoc requirements for investigative or other purposes

The post holder will conform to  all approvedquality initiatives which may be present within the hospital and ensure quality and best practice within their work at all times.

 

  1. Continuing Medical Education (CME)

5.1.  The Program

The postholder will attend approved CME session as required for maintenance of professional license

5.2.  Presentations

The post holder will take part in the program by presenting a topic at least on an annual basis

 

  1. 6.    Advisory

The post holder will provide pharmacy advise and guidance to colleagues as requested.

 

7     supervisory

        The post holder will provide professional supervision and guidance to  staff assigned to the department

    

4.   DECISION MAKING AUTHORITY:

Responsible for clinical desicion making in appropriate areas

5.   COMMUNICATIONS AND RELATIONSHIPS:

Internal:   

Pharmacy colleagues,  all medical colleagues, all nursing colleagues

External: 

As required by CMS or CMO with Ministry of Health, NHIP and TCIG representatives

6.    KNOWLEDGE, TRAINING AND EXPERIENCE REQUIRED FOR JOB

Education

Required

  • Minimum requirement-Bachelor of pharmacy degree from a recognized (accredited) university.
  • Current practice licence from country of recruitment
  • Evidence of ongoing participation in approved continuing education programmes
  • Current registration with TCI medical board

 

Experience

Required

  • At least five years Experience in hospital pharmacy practice
  • Sterile IV compounding and admixture
  • Oncology experience
