The Case Manager is charged with coordinating and facilitating patient care through assessment, evaluation, planning and implementation of care, based on patient’s needs.
PRIMARY DUTIES & RESPONSIBILITIES (included but not limited to):
1) Assess new clients by reviewing records and applications; identify clients’ desired outcomes of care.
2) Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
3) Coordinates with other disciplines the treatment plan by setting schedules and routines: coordinating services being provided; arranging resources, including transportation and escort.
4) Monitors clients’ response to treatment advocating for needed services, entitlements; and additional resources; intervening in crises and provide personal support.
5) Maintains clients’ records by reviewing case notes; logging events and progress according to MCG.
6) Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment outcomes.
7) Prepares clients’ discharge and coordinate discharge requirements; orienting and training family members; providing necessary resources and referrals.
8) Provide educational opportunities and resources for staff; evaluate treatment results in relation to learning.
9) Use of Continuous Quality Improvement (CQI) and Utilization Review (UR) principles to improve treatment of patients.
10) Efficient with EHR and databases analysis
11) Improves financial status by analyzing results; monitoring variances; identifying trends; recommending actions to management.
12) Meets budget by monitoring expenses; implementing cost-saving actions.
13) Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics; completing grant and subsidy applications.
14) Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
15) Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
16) Performs other related duties which are consistent with the role profile and may be required in specialty units
17) To collect data and provide reports on activities of the unit.
18) Be active in discharge planning Committee and the discharge planning process.
Minimum Requirements:
Educational/Training Requirements
v Bachelors of Science Degree in Nursing
v Certificate/ Diploma in Case Management.
v Certified Case Manager preferred
Experience
v Three years’ experience in case management
v Five (5) years nursing experience as a Registered Nurse with varied clinical experience.
Licenses/Certificates
v Unencumbered BVI Nursing License and current registration to practice

SALARY: $55,498.00 - $63,483.00 (Commensurate with qualifications and experience)
